Mary Romney, an ENT specialist at Scottsdale Sinus and Allergy Center, said on Instagram that sinus infections can, in rare cases, spread to the brain and cause serious conditions such as meningitis.

“Sinus infections can spread,” said Romney, according to Instagram. “It is rare. They do have the ability to spread into the brain. They can lead to a more serious infection like meningitis.”

According to the National Library of Medicine, acute bacterial sinusitis can infrequently lead to intracranial complications like meningitis. A retrospective study found that among 82 patients with complicated sinusitis, 21 developed meningitis, making it the most common intracranial complication observed. These cases often involved sphenoid sinusitis and were associated with pathogens like Streptococcus pneumoniae and Cryptococcus species.

Intracranial complications of sinusitis, including meningitis, are rare but serious. A study by the Rhinology Journal reported that meningitis accounted for 12% of intracranial complications in patients with acute bacterial rhinosinusitis. These complications often necessitate prompt medical intervention to prevent severe outcomes.

According to PubMed, the paranasal sinuses are adjacent to intracranial structures, and infections can spread through venous channels such as the facial and angular veins to the cavernous sinus. The “danger triangle” of the face, extending from the corners of the mouth to the bridge of the nose, has venous connections to intracranial venous sinuses. This pathway allows for possible retrograde spread of infection, which can result in complications such as cavernous sinus thrombosis and meningitis.
