Arizona and Florida, two states with some of the highest rates of uninsured children in the nation, have set goals to expand health insurance coverage for those 18 and younger. However, their approaches illustrate significant ideological differences regarding government subsidies for children's health insurance.

Joan Alker, executive director of Georgetown University's Center for Children and Families, commented on the situation: "It’s a tale of two states." She noted that this divergence reflects a broader reconsideration of government's role in family welfare post-COVID-19 pandemic. The differing policies also highlight key issues at stake in the 2024 presidential election.

In 2023, Republican-led legislatures in both states passed bills to expand their Children's Health Insurance Program (CHIP), which covers individuals under 19 from families earning too much to qualify for Medicaid. Subsequently, Florida's Republican Governor Ron DeSantis and Arizona's Democratic Governor Katie Hobbs signed these bills into law. While both laws increased income eligibility limits for CHIP programs, the similarities end there.

Arizona began enrolling newly eligible children in March and has aligned its policies with the Biden administration's Affordable Care Act-style protections. This includes eliminating annual and lifetime coverage limits and removing penalties for missed premium payments. Arizona suspended monthly premiums for its CHIP plan, KidsCare, in 2020 and is considering whether to reinstate them due to new federal rules preventing disenrollment for nonpayment.

"We’re trying to understand if the juice is worth the squeeze," said Marcus Johnson, a deputy director for Arizona’s Medicaid agency.

Conversely, Florida has not yet begun expanded enrollment and remains unique in challenging a Biden administration rule mandating continuous enrollment for children regardless of premium payment status. A judge dismissed Florida’s lawsuit on May 31 but allowed an appeal to federal regulators. The state's CHIP expansion awaits federal approval before new enrollments can commence.

Sara Lonardo from the Department of Health and Human Services emphasized: "No eligible child should face barriers to enrolling in CHIP or be at risk of losing the coverage they rely on."

Florida's expansion plan includes significantly raising premiums annually by 3%. State officials estimate an additional $90 million cost in its first full year while expecting $23 million from new premiums. However, compliance with federal provisions barring disenrollment could result in a $1 million monthly loss according to state estimates.

Public records obtained by the Florida Health Justice Project indicate that over 22,000 children have been removed from CHIP due to unpaid premiums since January 1st despite federal regulations against such actions.

DeSantis’ office did not respond to requests for comment about CHIP but described it as a “personal responsibility program” intended as a transition from Medicaid to private insurance.

For some residents like Emily Dent from Cape Coral, higher premiums pose financial challenges rather than promoting self-sufficiency. Her son James was disenrolled from Medicaid due to high family income but would qualify under Florida's proposed expansion—though at a steep $195 monthly premium.

“He has to have health insurance,” Dent said about her son who requires constant care due to Pallister-Killian syndrome. “But it’s going to drain my savings.”

Research indicates that even low premiums can deter families from maintaining CHIP coverage without significantly offsetting state costs. Matt Jewett from Arizona’s Children’s Action Alliance pointed out that federal contributions cover most program costs after deducting collected premiums.

"Premiums are more about an ideological belief that families need skin in the game," Jewett stated.

Twenty-two states currently charge CHIP premiums regardless of political leaning since its inception allowed such flexibility—a factor contributing significantly towards bipartisan support according Jennifer Tolbert at KFF.

Over time however governmental roles evolved notably following President Obama’s Affordable Care Act while Trump showed less interest during his tenure; contrastingly Biden administration policies favor easing access/retention particularly among low-income Americans.
