The Arizona Department of Health Services (ADHS) has announced that Avanti Senior Living in Peoria is now under new ownership and licensed as Spring Gardens Peoria, an Avista Senior Living Community. As of November 26, 2024, a provisional license was issued to Avista Senior Living Community, which will oversee the facility's operations.
Residents and their families have the option to remain at the facility. Avista Senior Living Community is coordinating with residents regarding the transition. ADHS expressed gratitude towards staff and partners for resolving issues with Avanti and pledged continued collaboration with Avista during the transition.
"We strive to ensure that ADHS Licensees receive the support they need to provide high-quality services," said an ADHS representative. The department will monitor progress closely to ensure a smooth transition and continuity of care. Residents and family members are encouraged to report any concerns about care quality during this period at avistaseniorliving.com.
ADHS licenses, regulates, and assists healthcare facilities in maintaining high standards of care in Arizona. The department occasionally revokes licenses when necessary corrective actions are not taken by licensees.
Last week, it was reported that Avanti Senior Living had advised residents to find alternative living arrangements without providing clear timelines or reasons for its license revocation. Despite having over a year to prepare for closure, Avanti reportedly downplayed the situation as a paperwork error.
ADHS had notified Avanti over a year ago about unresolved negligent practices leading to license revocation. Attempts were made by ADHS to negotiate a settlement agreement, but Avanti opted for judicial proceedings instead. Two courts upheld ADHS's decision, with an Administrative Law Judge noting "a blatant disregard for rules, procedures, and law" by the facility.
The timeline reveals that on August 14, 2023, ADHS issued a Notice of Intent to Revoke Avanti's license after multiple regulatory violations posing risks to resident health and safety. On May 28, 2024, an Administrative Law Judge affirmed this decision despite appeals from Avanti. A Superior Court denied further motions on November 4, effectively terminating the license.
